My deepest sympathies on your loss. You have experienced a parent's worse fear. I am not a medical professional; however, I am a skeptic when it comes to the medical community. If it was me, I would hire a malpractice attorney (or what ever you Brits call them) and request a complete copy of his medical records. Of particular focus should be the anesthesia cocktail they used to sedate your son. Many doctor's like including Versed (midazolam) in the mix because it helps the patient to relax and later forget the entire event.

"WARNING: Rapid intravenous (IV) use of midazolam can cause severe breathing problems which could cause brain damage or be fatal if left untreated. Therefore, when using this medication IV it must be used only under close medical supervision. Children, infants, elderly or weak patients are more sensitive to the effects of midazolam. Thus, lower doses should be used for these patients or when other sedating drugs are used with midazolam. Newborns must not be given midazolam by rapid IV injection."

Read the precautions and see if any apply to your son's cause of death. The anesthesiologist had control of your son's life the minute the IV was started. There is a drug to reverse the effects; but, you have to work fast.

In the US, Versed is the drug of choice for sedation during a colonoscopy. I refuse sedation for a colonoscopy and Versed is one of the reasons. The link below in sources should guide you on your search for the truth of what went wrong.

Litigation won't bring your son back; but, hopefully it will stop someone else from having to go through your pain. It's not about the money, it's about accountability.

first I want to pass you my heartfelt condolences, when we lose a loved one we need to know or at least believe that the medical profession has done everything human possible to take care of our precious loved one.Obviously people working within our health service are human beings and mistakes can occur, sadly though most doctors nurses and other so called health "care" professionals will never ever admit they have made a mistake most mistakes are covered up and if they come to light to the patient or their relatives HCP do all they can to make it nigh impossible for anyone to examine the facts,We have been programmed through the past sixty yrs to treat NHS staff and GPs as special cases, special people as if they are not accountable and as if we should accept without Q every thing they do or say as being right,The medical profession is no different to any other profession we have bad builders, vets, electricians, plumbers, hairdressers etc etc medics are no different some are good others are bad but we are conditioned to believe they are "called to tend the sick" special people WRONG it is time we realised not all are killers like Harold Shipman or Beverley Allitt but there are manyhundreds who treat or loved one with much less that we should expect from their profession.
I hope and pray that the truth surrounding your beloved son which has not been admitted by those responsible will come to light and those responsible will be found and that they get the judgement they deserve but I fear that you will not get that this side of heaven, they cannot hide the truth from God, I pray for you and your family and for your son.I hope God will grant you a measure of peace and the knowledge that he is now at peace.
References :
one who has seen loved ones dealt with by an uncaring and "superior" profession thankfully afew were kind and caring but sadly too few.

I'm sorry to hear about your son. You have my deepest sympathy and condolences. I nor anyone on this site is a medical expert though I do think it sounds as if your sons case was handled badly. The medical records certainly shouldn't have been mixed up like they were. I cannot give a judgement as to whether there was medical negligence and nor can anyone else who is not medically qualified unfortunately. I hope that the report, which does seem to conclude they could not have known about the portal hypertension will bring some closure and allow you to lay your son to rest.
References :
My heart goes out to you; it really does. Of all the questions I have ever read on here, this is the saddest. I have tears in my eyes.

What can I say? Keep fighting if you can, if there is anything else you can do, or anywhere left to go legal-wise. You would have thought they should have known earlier, yes.

But if you hit a brick wall with the establishment, rest assured that someone higher than us saw it all and with endless love in his home above all will be righted there.

God bless you and comfort you. (((((hugs)))))
References :
im sooo sorry. life isnt fair. doctors arent gods and i know that if i didnt go to hospital i wouldnt be as sick as i am now. but thats the way life is. you roll the dice, you do what is best at the time and you take your chances, its not fair, but thats the way it is. your son would want you to live your life to the fullest whatever happened. nothing will change things now. your son is gone, my health is gone. just mourn and move on. thats the way it is. no money will replace your son or my health. im sorry.
